Registration

Registration Process:

  1. Provide an application to the parent or guardian of a prospective participant. 
    • Paper copy available at ICC upon request
    • Printable PDF available on website or via email
  2. Contact the ICC Director to schedule an appointment for application submission (no drop-ins). Contact via email: inspirecontinuingcare@gmail.com or phone/text: (309) 714-8647. To be considered “complete”, a registration form must be:
    • Filled out in its entirety (NO blanks)
    • Include $25 registration fee (non-refundable)
    • Include all supporting documents/references:
      • Please include a copy of applicant’s 
        • Exit/Transition IEP
        • Summary of Performance document (this may be a part of the exit/transition IEP) 
      • Please provide name and contact information of three (3) reference individuals who have had extended interaction with applicant in a community-based  environment (school, work, church, club . . . a non-home environment) 
  3. ICC Director reviews registration form and supporting documentation. Prospective participant is either approved for ICC visit days or it is determined that ICC is not a good fit for the prospective participant because (reason will be provided)
  4. ICC Director schedules (2) unaccompanied visit days for prospective participant: 
    • On-site visit 
    • In community visit
  5. ICC Director and Staff review notes from the (2) visit days and provide feedback to the ICC Board of Directors. 
  6. ICC Board of Directors reviews documentation and considers feedback from the  ICC Director/Staff prior to voting whether to approve or deny the application of a prospective participant. 
  7. ICC DIrector will notify parent/guardian of the final decision of the ICC Board of Directors.
